Brief Description
SecondChance App helps patients find solutions for hard-to-diagnose and hard-to-treat illnesses. Patients are matched with the best physician to meet their needs and are expected to ha a journey with several touch points, not a one-time transaction. The experience is built to educate, empower, and ultimately improve the health of patients who have not previously been well-served by the health care system.
Business Goals
- Let a patient see one or more doctors for a second opinion, without giving up access to their primary care provider.
- Diagnose and effectively treat patients more quickly.
- Reduce the amount of resources they use by slow diagnoses and ineffective or poorly tailored treatments.
- Improve customer service in the healthcare industry.
App Objectives
- Track what happens on the patient’s medical journey, soliciting feedback on their treatment, diagnosis, and level of satisfaction.
- Prioritize patient experience and avoid the dismissing of symptoms that are not common or widely observed.
- Connect patients with doctors that are familiar with their symptoms.
- Help ease patients’ worries when experiencing illness and not knowing what the cause of the problem is.
Assumptions
Ontario’s health care system is centralized and all PCPs and Specialists can access patient’s records. Using a patient’s health card number, doctors can access a patient’s full medical history.
